Year 6 Dental Survey 2023

The Dental Team from the Manchester Community Dental Service are coming into your school to undertake a dental survey. They will be looking at the teeth of Year 6 children.

If you receive a letter inviting your child to take part, please complete the form and return to school as soon as possible. An information leaflet will be provided that explains about the dental survey.

Unfortunately, the Dental Team are only able to see a certain number of children at this time. The children to be seen are selected at random by The Dental Team from the class lists provided by your school. The school will be provided with a list of names of children to be seen by the Dental Team.
